پیش‌زمینه و هدف: اختلال در سیستم عصبی خودکار موجب عوارض ثانویه در سالمندان با بیماری دیابتی نوع دو می‌شود. هدف از پژوهش حاضر تأثیر تمرین راه رفتن بر مقاومت انسولین، نیمرخ لیپیدی و تغییرپذیری ضربان قلب در سالمندان دارای دیابت نوع 2 بود.
مواد و روش کار: روش پژوهش حاضر از نوع نیمه تجربی با طرح پیش‌آزمون - پس‌آزمون بود. جامعه آماری پژوهش حاضر را کلیه سالمندان دیابتی نوع دو شهر تهران تشکیل دادند. 30 مرد جودوکار به روش نمونه‌گیری در دسترس و به‌طور تصادفی در 2 گروه، گروه اول (تمرین راه رفتن)، گروه دوم (کنترل) تقسیم شدند. متغیرهای خونی و قلبی قبل (24 ساعت قبل) و بعد از اجرای برنامه‌های تمرینی (72 ساعت) از آزمودنی‌ها گرفته شد. پروتکل تمرینی شامل یک برنامه پیاده روی 8 هفته‌ای بود که با استفاده از مقاله کائو (2007) و همچنین پیشنهادات متخصصین در رابطه با طراحی و اجرای برنامه تمرینی در افراد سالمند و دیابتی که سه روز تمرین در هفته و شدت 35درصد حداکثر اکسیژن مصرفی یا 45درصد درصد ضربان قلب پیشنهاد کرده بودند طراحی شد.
یافته‌ها: نتایج آزمون بین گروهی نشان داد کلسترول (020/0=P) و مقاومت انسولینی (006/0=P) در دو گروه تمرینی و کنترل اختلاف معنی‌داری داشت. SDNN (013/0=PrMSSD (005/0=PHF Power (001/0>P) و LF/HF Ratio (001/0=P) در دو گروه تمرینی و کنترل اختلاف معنی‌داری داشت. نتایج آزمون درون گروهی نشان داد مقاومت انسولینی (009/0=P) و LF/HF Ratio (020/0=P) در گروه تمرینی در مرحله پس‌آزمون در مقایسه با پیش‌آزمون کاهش معنی‌داری داشت. SDNN (003/0=PrMSSD (004/0=P) و HF Power (004/0=P) در گروه تمرینی در مرحله پس‌آزمون در مقایسه با پیش‌آزمون افزایش معنی‌داری داشت.
بحث و نتیجه‌گیری: با توجه به نتایج تحقیق می‌تواند نتیجه گرفت که یک دوره تمرین راه رفتن می‌تواند منجر به بهبود مقاومت انسولینی و تغییرپذیری ضربان قلب در سالمندان با بیماری دیابتی نوع دو شود. لذا این تمرینات به‌عنوان تدبیری جهت کاهش عوارض قلبی-عروقی در سالمندان با بیماری دیابتی نوع دو توصیه می‌شود.
