fa ارزیابی کیفیت داده‌های کدگذاری شده در سیستم ثبت ترومای دانشگاه علوم پزشکی ارومیه EVALUATION OF THE QUALITY OF CODED DATA IN THE TRAUMA REGISTRY SYSTEM OF URMIA UNIVERSITY OF MEDICAL SCIENCES اطلاع رسانی پزشکی اطلاع رسانی پزشکی پژوهشي(توصیفی- تحلیلی) Research پیش‌زمینه و هدف: سیستم ثبت تروما به‌عنوان ابزاری برای شناسایی عوامل خطرزا، ارزیابی کیفیت خدمات مراقبت و انجام تحقیقات ایجاد شده است. جهت نیل به این مهم، ثبت کامل اطلاعات و کدگذاری دقیق آن‌ها، امری حیاتی است. هدف از انجام این مطالعه، ارزیابی کیفیت داده‌‌‌های کدگذاری در سیستم ثبت ترومای دانشگاه علوم پزشکی ارومیه است. مواد و روش کار: این پژوهش توصیفی-مقطعی بود که در سال 1402 انجام شد. جامعه پژوهش شامل 400 پرونده پزشکی بود که از طریق نمونه‌گیری تصادفی انتخاب شدند. ابزار گردآوری داده‌ها، چک‌لیست محقق ساخته بود که روایی و پایایی آن تائید شد. دادهها با استفاده از نرمافزار SPSS تحلیل گردید. یافته‌ها: درمجموع برای 400 پرونده ترومایی به ترتیب 1001 و 1050 کد توسط کدگذار سیستم و خبره ثبت گردید. نتایج به‌دست‌آمده 95درصد کامل بودن کدها را نشان داد. صحت کدگذاری تشخیص‌ها در سطح سه کاراکتر اول (81درصد) چهارم (61درصد) و پنجم (30درصد) و برای کدگذاری علل خارجی در سطح سه کاراکتر اول (62درصد) چهارم (35درصد) و پنجم (28درصد) بود. پایایی کدهای اختصاص داده‌شده توسط کدگذار برای تشخیص و علت خارجی در سطح سه کاراکتر اول 8/91%=κ، کاراکتر چهارم 3/84%= κ و کاراکتر پنجم 35%k= به دست آمد. بحث و نتیجه‌گیری: نتایج این مطالعه نشان داد که داده‌های کدگذاری شده ازلحاظ کامل بودن و پایایی کدها مطلوب است. در ارزیابی صحت کدها در سطح سه کاراکتر اول مطلوب اما در سطح کاراکتر چهارم و پنجم نامطلوب است. بنابراین ضروری به نظر می‌رسد جهت ارتقای کیفیت کدگذاری سیستم ثبت تروما، ارزیابی‌های مداوم از صحت کدگذاری انجام یافته و بازخوردهای لازم به کدگذاران ارائه شود. همچنین جهت افزایش مهارت و توانمندسازی کدگذاران، آموزش‌های ضمن خدمت برگزار شود. Background & Aims: The trauma registry system was established as a tool to identify risk factors, assess service quality, and conduct research. To achieve this goal, complete recording of information and accurate coding are vital. The purpose of this study is to evaluate the quality of coded data in the trauma registry system of Urmia University of Medical Sciences. Materials & Methods: This descriptive-cross-sectional and retrospective research was conducted in 2023. The research population consisted of 400 medical records, selected through random sampling. The data collection tool was a checklist developed by the researcher, whose validity and reliability were confirmed. The data were analyzed using SPSS software. Results: In total, 1001 and 1050 codes were registered by the system coder and the expert, respectively, for 400 cases. The obtained results showed a 95% completeness of the codes. The accuracy of coding diagnoses was at the level of the first three characters (81%), the fourth (61%), and the fifth (30%), and for coding external causes at the level of the first three characters (62%), the fourth (35%), and the fifth (28%). The reliability of the codes assigned by the coder in the first and second stages was found to be κ=91.8% for the first three characters, κ=84.3% for the fourth character, and κ=35% for the fifth character. Conclusion: The results of this study showed that the coded data are favorable in terms of completeness and reliability of codes. In evaluating the accuracy of codes, at the level of the first three characters, it is favorable, but at the level of the fourth and fifth characters, it is unfavorable.
