بررسی الگوی مقاومت آنتی بیوتیکی باسیلهای گرم منفی، جدا شده از نمونههای کشت ادرار مرکز آموزشی و درمانی امام رضا(ع)-کرمانشاه

چکیده
پیش زمینه و هدف: مقاومت نسبت به آنتی بیوتیک ها در میان باکتری های پاتوژن به عنوان یک مشکل در سراسر جهان مورد توجه است. تعیین الگو های مقاومت آنتی بیوتیکی در مورد باکتری های بیماری زای شایع جهت هدایت درمان های امپریکال (تجربی) و اختصاصی بر علیه یک پاتوژن خاص حایز اهمیت است. به همین دلیل اقدام به تعیین الگوی مقاومت آنتی بیوتیکی باکتری های گرم منفی در نمونه های ادرار نمودیم.
مواد و روش کار: از کلیه نمونه های ادرار ی که جهت کشت به آزمایشگاه مرکز آموزشی درمانی امام رضا (ع)، وابسته به دانشگاه علوم پزشکی کرمانشاه طی مدت 12 ماه (فروردین تا اسفند 85) ارسال شد، در محیط های مکانگی آگار، بلاد آگار و محـیط های کشت افـتراقی و تشخیصی باکتری های گرم منفی کشت گردید و پس از آن، الگوی آنتی بیوگرام این باکتری با روش انتشار دیسک Kirby-Bauer و هاله عدم رشد، طبق استاندارد های کمیته ملی برای
آزمایشگاه های بالینی NCCLS)) مورد بررسی قرار گرفت.
یافته ها: از تعداد 10492 نمونه ادرار کشت داده شده 1815 مورد کشت مثبت (29/17 درصد) بدست آمده از این تعداد 1239 مورد باسیل گرم منفی
(2/68 درصد) که شایع ترین جرم آن E.coli با 3/66 درصد بود. به لحاظ حساسیت آنتی بیوتیکی باسیل های گرم منفی بیشترین حساسیت را به ایمی پنم (9/67 درصد) و بیشترین مقاومت را به آمپی سیلین (6/91 درصد ) نشان دادند. در نمونه های E.coli بیشترین مقاومت نسبت به آمپی سیلین (4/91 درصد) و کوتریموکسازول 3/61 درصد دیده شد. در ضمن مشخص گردید که پسودوموناس تنها در برابر ایمی پنم (100 درصد) و سیپروفلوکساسین (69 درصد) حساسیت بالایی نشان می دهد و تقریباً به سایر آنتی بیوتیک ها مقاوم است.
بحث و نتیجه گیری: با توجه به تفاوت نمای آنتی بیوگرام در مناطق مختلف جغرافیایی استفاده از الگوی مقاومت آنتی بیوتیکی منطقه ای در درمان تجربی و اختصاصی بیماران لازم می باشد.

Abstract
Background & Aims: Antimicrobial resistance among bacterial pathogens is a worldwide problem. The selection of initial treatment is made on an empiric basis and susceptibility testing is important to modify empirical therapy, especially for treatment of common bacterial pathogens. The aim of the present study was to determine the antimicrobial susceptibility of gram negative pathogens found in urine cultures.
Materials & Methods: The study samples were all urine specimens submitted for culture in central laboratory of Imam Reza Hospital during a 12-month period during 2006-2007. All of the specimens were cultured on MacConky agar, blood agar and differential media for gram negative bacteria. After diagnosis and isolation of gram negative bacteria, proper inoculum was used for susceptibility testing. Susceptibility testing was done by disk diffusion test (Kirby-Bauer method) and interpreted according to NCCLS recommendation.
Results: Of 10492 cultured urine samples, 1815 (17.29%) specimens had positive results, 1239(68.2%) cases were positive for gram negative bacteria and the most common pathogen was E.coli (66.3%). Antibiotic susceptibility results revealed that gram negative bacterial have the greatest susceptibility to imipenem (67.9%). The E.coli antibiogram profile showed the greatest resistance to ampicillin (91.6%) and cotrimoxazole (61.3%). Pseudomonas Sp. was an exception that resisted to all antibiotics except for imipenem (100%) and ciprofloxacin(69%).
Conclusion: According to the results, it is apparent that an antimicrobial susceptibility test of bacteria differs by geographic region therefore, in order to confirm a diagnosis, Antibiotic resistance patterns of a community and empirical therapy must be considered.
