fa تعیین نقش سطح سرمی PAPP-A مادر در پیش‌بینی سرانجام حاملگی DETERMINATION OF THE ROLE OF MATERNAL SERUM PAPP-A LEVEL IN PREDICTING PREGNANCY OUTCOME زنان و زایمان زنان و زایمان پژوهشي(توصیفی- تحلیلی) Research پیش‌زمینه و هدف: با توجه به عوارض بارداری ازجمله سقط، پره اکلامپسی، زایمان زودرس و مرگ داخل رحمی شناسایی مادرانی که در معرض عوارض بارداری هستند، می‌تواند به کنترل و بهبود نتایج بارداری کمک نماید. هدف از این مطالعه در زمینه مراقبت‌های پری ناتال رسیدن به نتایج خوب بارداری بودکه سلامت مادر و نتایج جنینی خوب را مدنظر قرار دهد. مواد و روش‌ها: در مطالعه مورد-شاهدی حاضر، 400 زن باردار از بین زنان بارداری که که در نیمه دوم 1398 و نیمه اول 1399 به درمانگاه‌های پریناتال بیمارستان مطهری ارومیه مراجعه کرده بودند، و همچنین مادرانی که در 3 ماهگی اول بارداری بودند، انتخاب شده و وارد مطالعه شده و  آنها به دو گروه 200 نفری تقسیم شدند.گروه مورد شامل 200 مادر بود که سطح سرمی PAPP-A در آنها کم‌تر از 4/0 بود و آسپیرین هم مصرف نکرده بوند. گروه شاهد هم شامل 200 مادر بود که سطح سرمی PAPP-A در آنها بیشتر از 4/0 بود. عوارض بارداری شامل پره‌اکلامپسی، فشارخون حاملگی، دکولمان جفت، محدودیت رشد جنین و مرگ داخل رحمی بین گروه‌ها مقایسه شد. یافته‌ها: نتایج این تحقیق  نشان داد، پره کلامپسی بعلت PTL (زایمان زودرس) (001/0=P) و همچنین بعلت IUFD (مرگ داخل رحمی جنین) (049/0=P) ارتباط معناداری با سطح سرمی پایین PAPP-A داشته، درحالی‌که سطح سرمی پایین PAPP-A با پره‌اکلامپسی، IUGR (محدودیت رشد داخل رحمی) و فشارخون حاملگی ارتباط معناداری نداشت. بحث و نتیجه‌گیری: اندازه‌گیری سطح سرمی PAPP-A به‌تنهایی نمی‌تواند پیش‌بینی کننده عوارض دوره حاملگی مانند پره‌اکلامپسی، IUGR، و زایمان زودرس باشد و به نظر می‌رسد که باید به همراه سایر اندکس‌های بالینی و پاراکلینیکی پیش‌بینی کننده این عوارض در مادران باردار مورد ارزیابی قرار گیرد. Background & Aims: Considering the complications of pregnancy such as abortion, preeclampsia, preterm delivery and intrauterine death, identifying mothers who are at risk of pregnancy complications can help control and improve pregnancy outcomes. The goal of this perinatal care study was to achieve good pregnancy outcomes that considered maternal health and good fetal outcomes. Materials & Methods: In the present case-control study, 400 pregnant women who were referred to the prenatal clinics of Motahari Hospital in Urmia in the second half of 1398 and the first half of 1399, as well as mothers who were in the first 3 months of pregnancy, were selected and included in the study. They were divided into two groups of 200. The case group consisted of 200 mothers with a serum PAPP-A level of less than 0.4 and no aspirin. The control group also included 200 mothers with serum PAPP-A levels higher than 0.4. Pregnancy complications including preeclampsia, gestational hypertension, placental abruption, fetal growth restriction and intrauterine death were compared between groups. Results:  The results of this study showed that preeclampsia due to PTL (Preterm Labor) (P = 0.001) and also due to IUFD (Intrauterine Fetal Death) (P = 0.049) had a significant relationship with low serum PAPP-A, while Low serum PAPP-A level was not significantly associated with preeclampsia, IUGR (intrauterine growth restriction) and gestational hypertension.
