تاثیر 12 هفته تمرین در آب بر مقادیر RBP4، مقاومت انسولینی و تغییرات آنزیمهای کبدی زنان مبتلا به دیابت نوع 2
The effect of 12 weeks of Aqua training on RBP4, insulin resistance, and liver enzymes in women with type 2 diabetes

<strong>چکیده</strong><br>
<strong>مقدمه:</strong> ریتونول متصل به پروتئین 4 (<span dir="LTR">RBP4</span>) به عنوان یک آدیپوکاین در تنظیم عملکرد انسولین و متابولیسم گلوکز شرکت دارد. سطح سرمی <span dir="LTR">RBP4</span> به عنوان یکی از عوامل موثر در بروز اختلال در تحمل گلوکز و پیامد آن در دیابت مشخص شده است. مطالعات نشان داده است که تمرینات ورزشی سبب بهبود عوامل سندرم متابولیک و بیماری­های کبدی می­شود. اما هنوز آثار تمرین در آب بر فاکتورهای کبدی در بیماران دیابتی ناشناخته است. لذا، هدف پژوهش حاضر بررسی تاثیر 12 هفته تمرین در آب بر مقادیر <span dir="LTR">RBP4</span>، مقاومت انسولینی و تغییرات آنزیم های آسپارتات آمینوترانسفراز <span dir="LTR">(AST)</span> و آلانین آمینوترانسفراز (<span dir="LTR">ALT</span>) کبدی زنان مبتلا به دیابت نوع 2 می­باشد.<br>
<strong>مواد وروش ها:</strong> 44 نفر از بین زنان مبتلا به دیابت نوع 2 با دامنه سنی 55 تا 60 سال، به روش نمونه­گیری در دسترس انتخاب و به دو گروه کنترل و تمرین تقسیم شدند. گروه تمرین به مدت 12 هفته (سه جلسه در هفته، هر جلسه 60 دقیقه) تمرینات در آب را با شدت 75-60 درصد ضربان قلب بیشینه انجام دادند. برای تجزیه و تحلیل متغییرهای پژوهش از آزمون آماری <span dir="LTR">t</span> وابسته و مستقل در سطح معنی­داری (<span dir="LTR">p<0/05</span>) استفاده شد.<br>
<strong>یافته­ ها:</strong> نتایج آزمون آماری نشان داد 12 هفته تمرین در آب به طور معنی داری مقادیر <span dir="LTR">RBP4</span>، مقاومت انسولینی و آنزیم­های <span dir="LTR">AST</span> و <span dir="LTR">ALT</span> را کاهش داد و همچنین پروفایل لیپیدی را بهبود بخشید. نتایج آزمون <span dir="LTR">t</span> مستقل نشان داد که بین مقادیر <span dir="LTR">RBP4</span>، مقاومت انسولینی، آنزیم های کبدی و همچنین پروفایل لیپیدی بین گروه شاهد و تمرینی تفاوت معنی­داری وجود دارد. (<span dir="LTR">p<0/05</span>)<br>
<strong>نتیجه ­گیری</strong>: بر اساس نتایج پژوهش حاضر تمرین در آب سبب کاهش <span dir="LTR">RBP4</span> شده که این کاهش با بهبود مقاومت به انسولین همراه است و با تغییرات آنزیم های کبدی و متابولیکی، اثر شبه انسولینی روی زنان دیابتی دارد.<br>

<strong><em>Background & Aims</em></strong>: Retinol-binding protein 4 (RBP4) as an adipokine is involved in regulating insulin function and glucose metabolism. Serum levels of RBP4 have been identified as one of the most effective factors in impaired glucose tolerance and diabetes. Studies have shown that exercise improves metabolic syndrome and hepatotoxicity. But the effects of Aqua training on liver Enzymes in diabetic patients are still unknown. Therefore, the aim of this study was to investigate the effect of Aqua training on RBP4, insulin resistance, and aspartate aminotransferase (AST) and alanine aminotransferase (ALT) enzymes in women with type 2 diabetes. <br>
<strong><em>Materials & Methods</em></strong><strong>:</strong> Forty-four women with type 2 diabetes (57.83± 0.42 years) were selected through available sampling and divided into two groups of control and training. The training group performed the exercise in the water at 60-75% of maximum heart rate for 12 weeks (three sessions per week, 60 minutes each session). Data were analyzed with dependent and independent t-test at the significant level of p≤0.05.<br>
<strong>Results:</strong> The results show that 12 weeks of Aqua training significantly reduced RBP4, insulin resistance, and AST and ALT, and improved lipid profiles (p ≤0.05). Also, the result of Independent t-test showed that there is a significant difference between training groups with control in variables of RBP4, insulin resistance, liver enzymes and lipid profiles (p ≤0.05).<br>
<strong>Conclusion:</strong> Based on the results of the present study, the Aqua training decrease RBP4, that it is associated with improved insulin resistance. Also 12 weeks of Aqua training has an insulin-like effect on diabetic women with changes in liver and metabolic enzymes.
