مقایسه حافظه کاری در میان بیماران سایکوز ناشی از مصرف مت آمفتامین، بیماران مبتلا به هپاتیت C و بیماران مصرف کننده مت آمفتامین مبتلا به هپاتیت C

<p dir="RTL"><strong>پیشزمینه و هدف: </strong>در پی تغییرات الگوی مصرف مواد در ایران و تغییر پیامدهای آن بهویژه آسیبهای شناختی پایدار حاصل از مصرف آنها، مطالعه حاضر باهدف مقایسه حافظه کاری در بیماران مبتلا به هپاتیت <span dir="LTR">C</span>، بیماران سایکوز ناشی از سوءمصرف متآمفتامین و بیماران سایکوز ناشی از سوءمصرف متآمفتامین که مبتلا به هپاتیت <span dir="LTR">C</span> هستند، انجام شد.</p>
<p dir="RTL"><strong>مواد و روش کار: </strong>در پژوهش علّی-مقایسهای حاضر، تعداد 90 نفر واجد معیارهای ورود به پژوهش از بیمارستان رازی تهران و مرکز تحقیقات ایدز دانشگاه تهران به شیوهی در دسترس انتخاب شدند. آزمون <span dir="LTR">N-back</span> (کرچنر، 1958) برای پژوهش فعلی به کار گرفته شد. جهت تحلیل دادهها از شاخصهای مرکزی و پراکندگی و همچنین تحلیل واریانس چندمتغیری و آزمون تعقیبی توکی استفاده گردید.</p>
<p dir="RTL"><strong>یافتهها: </strong>بین سه گروه در مؤلفههای حافظه کاری (نمره کلی=01/0<span dir="LTR">p<</span>، پاسخ نادرست و زمان واکنش= 001/0<span dir="LTR">p<</span>) تفاوت معنیداری وجود داشت. با توجه به نتایج آزمون تعقیبی، میانگین نمره کلی برای بیماران سایکوز ناشی از سوءمصرف مت آمفتامین بهطور معنیداری نسبت به دو گروه دیگر کمتر، میانگین پاسخ نادرست آنها نسبت به دو گروه دیگر بیشتر، همچنین میانگین زمان واکنش این گروه نسبت به دو گروه دیگر کمتر بود.</p>
<p dir="RTL"><strong>بحث و نتیجهگیری: </strong>مصرف طولانیمدت متآمفتامین از طریق ایجاد آسیبهای عصب-روانشناختی در نواحی مغزی، سبب اختلال در حافظه کاری میشود که موجب نقص در کارکردهای اجرایی، توجه و ... میشود. لذا شناسایی این عوامل میتواند علاوه بر درک بهتر مشکل، کاهش عودها، بستری شدنهای مکرر و سایر پیامدهای منفی آنها، به ارائهی راهکارهای مختلف درمانی منجر شود.</p>
<p dir="LTR"><a name="_Toc416081061"><strong><em>Background & Aims</em></strong></a>: Following the changes in substance abuse pattern and its consequences in Iran, especially constant cognitive impairments resulting from their use, this study aimed to compare working memory in psychotic patients who use methamphetamine and patients with hepatitis C and methamphetamine users with hepatitis C virus.</p>
<p dir="LTR"><strong><em>Materials & Methods</em></strong>: In this casual-comparative study, a total of 90 patients of Razi hospital in Tehran and AIDS Research Center at Tehran University who met the inclusion criteria were selected through convenient sampling. The n-back test (Kirchner, 1958) was used in the current study. Data were analyzed by the central and dispersion indexes and the Multivariate Analysis of Variance as well as the Tukey test.</p>
<p dir="LTR"><strong><em>Results</em></strong>: There was a significant difference between the three groups in working memory components (overall score = p < 0.01, incorrect answer and reaction time = p < 0.001). According to the results of the post hoc test, the average of the overall score for psychotic patients who use methamphetamine was significantly less and the average of the incorrect answer was more than the other two groups, also the average reaction time was lower than the other two groups.</p>
<p dir="LTR"><strong><em>Conclusion</em></strong>: Long-term use of methamphetamine causes impairment in working memory through creating neuropsychological damage in brain regions, which in turn leads to deficits in executive functions, attention and so on. Therefore, in addition to help in better understanding of the problem, reducing relapses, frequent hospitalizations and other negative outcomes, identifying these factors could lead to different therapeutic strategies.</p>
