fa بررسی ویژگی‌های دموگرافیک و بالینی در کودکان با صرع پایدار بستری‌شده در بیمارستان مطهری ارومیه از سال 1390 تا 1397 EVALUATION OF CLINICAL AND DEMOGRAPHIC FEATURES OF STATUS EPILEPTICUS AT URMIA MOTAHARI PEDIATRICS HOSPITAL DURING 2011-2019 کودکان (عمومی) کودکان (عمومی) پژوهشي(توصیفی- تحلیلی) Research پیش‌زمینه و هدف: تشنج یک بیماری نورولوژیک مزمن و یک اختلال سیستم عصبی ناشی از تخلیه الکتریکی شدید و ناگهانی سلول‌های عصبی است. صرع پایدار طبق تعریف، تشنجی است که به‌صورت مداوم یا متناوب بدون این‌که به حالت پایدار برگردد برای حداقل 30 دقیقه طول می‌کشد و در صورت عدم‌مداخله سریع درمانی منجر به عوارض جدی در بیمار می‌گردد. این مطالعه باهدف بررسی ویژگی‌های دموگرافیک و بالینی در کودکان با صرع پایدار بستری‌شده در بیمارستان مطهری ارومیه از سال 1390 تا 1397 طراحی و اجرا شده است. مواد و روش کار: پژوهش حاضر یک پژوهش توصیفی _گذشته‌نگر هست. معیارهای ورود به این مطالعه شامل تشخیص صرع پایدار و سن کمتر از 18 سال است. بیمارانی که اطلاعات ناقصی داشتند از مطالعه کنار گذاشته شدند. سابقه ابتلا به صرع در بیمار و بستگان درجه یک و اطلاعات دموگرافیک و بالینی بیمار در چک‌لیست ثبت و جمع‌آوری شد. یافته‌ها: نتایج این مطالعه نشان داد میانگین سنی و وزن تولد بیماران برابر با 49/37 ماه و 3/3064 گرم بود. 47 درصد بیماران سابقه قبلی صرع، 3/28 درصد بیماران، والدینی با رابطه فامیلی، 3/18 درصد سابقه فامیلی صرع در اقوام درجه یک،1/51 درصد سابقه بیماری نورولوژیک و 7/ 8 درصد نیز سابقه بیماری غیرنورولوژیک داشتند. جنسیت پسر فراوانی بیشتری از دختران داشت. نوار مغزی 9/32 درصد غیر نرمال و نشان‌دهنده وجود امواج صرعی بود. اکثر بیماران تب‌دار 9/74 درصد و غالب تشنج به‌صورت جنرالیزه 6/82 درصد بود. سی‌تی‌اسکن مغزی در 2/ 14 درصد و ام آرای مغزی در 4/6 درصد بیماران یافته‌های پاتولوژیک داشت میزان مورتالیتیه در بین این بیماران برابر با 3/12 درصد بود. بحث و نتیجه‌گیری: طبق نتایج مطالعه حاضر افراد فوت‌شده به‌طور معنی‌داری سن کمتر نسبت به افراد ترخیص شده داشتند. همچنین جنسیت مذکر بیشتر از مؤنث و تب یافته شایع در بیماران بود. Background & Aims Seizure is a chronic neurological disease and a disorder of the nervous system caused by severe and sudden electrical discharge of neural cells. Status epilepticus is defined as any generalized tonic-colonic seizures lasting more than 5 minutes, any focal seizures lasting more than 10 minutes, and recurrent seizures without returning to baseline between periods of seizures. This study was designed and conducted to investigate the demographic and clinical characteristics of children with status epilepticus admitted to Motahari Hospital in Urmia during 2011-2019. Materials & Methods: This is a descriptive-retrospective study. All the files having the following criteria were included in the study: having a full record of patients' data, all the paraclinical studies performed, having a final diagnosis of status epilepticus, and having the age less than 18 years. History of epilepsy in the patient and first-degree relatives, and the patient's demographic information were recorded in the checklist. Results: The results of this study showed that the mean age and birth weight of patients with status epilepticus in Urmia Shahid Motahari Hospital was 37.49 months and 3046.30 grams, respectively. About 47% of patients had a previous history of epilepsy, 28.3% of parents were relative, 18.3% had a family history of epilepsy, 51.1% had a positive history of neurological disorder, and 8.7% had a positive history of non-neurological disorder. Boys were affected more than girls. EEG abnormality was the most common abnormal paraclinical finding (32.9%). Most of the patients had fever (74.9%) and the majority of seizures were manifested as generalized ones (82.6%). The mortality rate among these patients was 12.3%. Conclusion: According to the results of the present study, the deceased patients were significantly younger than the discharged individuals.
