اثر عملکرد مکانیکی رحم و دهانه آن بر زایمان زودرس زنان باردار: مطالعه مبتنی بر تصاویر پزشکی بیمار

<strong><span style="font-family:Nazanin;"><span style="font-size:9.5pt;">پیشزمینه و هدف:</span></span></strong><span style="font-family:Nazanin;"><span style="font-size:9.5pt;"> زایمان زودرس یک فرآیند پیچیده و متأثّر از چندین عامل است که در آن نارسایی گردن رحم نقش مهمی در برخی بیماران دارد. در بیشتر مدت بارداری، عملکرد مکانیکی مناسب دهانه رحم برای حفظ جنین در رحم لازم است. نرم شدگی پیش از موعد دهانه رحم و کوتاه شدن دهانه رحم دو دلیل عمده زایمان زودرس هستند. هدف این تحقیق بررسی اثر</span></span> <span style="font-family:Nazanin;"><span style="font-size:9.5pt;">نرم شدن دهانه رحم و تغییر شکل کیسه آمونیوتیک</span></span> <span style="font-family:Nazanin;"><span style="font-size:9.5pt;">بر </span></span><span style="font-family:Nazanin;"><span style="font-size:9.5pt;">عملکرد مکانیکی دهانه رحم در اثر محیط مکانیکی اندام است.</span></span><br>
<strong><span style="font-family:Nazanin;"><span style="font-size:9.5pt;">مواد و روش کار</span></span></strong>: <span style="font-family:Nazanin;"><span style="font-size:9.5pt;">مدلسازی سهبعدی رحم، دهانه رحم و غشاء جنین انسان باردار بر اساس تصویربرداری سهبعدی رزونانس مغناطیسی، برای تحلیل عملکرد مکانیکی رحم و دهانه رحم تحت بار فیزیولوژیکی بارداری انجام شده است. برای توصیف بافت کلاژنی رحم و دهانه رحم در این تحقیق از ماده سازنده کامپوزیت هایپرالاستیک، ماده زمینه نئوهوکین با فیبرهای با توزیع پیوسته تصادفی نئوهوکین، استفاده شده است. مقایسه تأثیرگذاری تغییر شکل دهانه رحم بر روی زایمان زودرس با استفاده از در نظر گرفتن دو نوع فیبر، باردار، نرم و تغییر شکل یافته، و غیرباردار، سفت و بدون تغییر شکل انجام پذیرفت. همچنین اثر هندسه کیسه آمونیوتیک با فرض دو هندسه متفاوت برای کیسه آمونیوتیک، تغییر شکل یافته و بدون تغییر شکل و سالم بررسی شده است. بررسی رفتار تغییر شکل بافت در </span></span><span style="font-family:Nazanin;"><span style="font-size:9.5pt;">اثر</span></span><span style="font-family:Nazanin;"><span style="font-size:9.5pt;"> تنش ایجادشده، </span></span><span style="font-family:Nazanin;"><span style="font-size:9.5pt;">تغییرات</span></span><span style="font-family:Nazanin;"><span style="font-size:9.5pt;"> هندسه اندام و اثرات متقابل فیزیکی رحم، گردن رحم و غشای جنین با بهکارگیری روش اجزای محدود و هندسه خاص بیمار بر اساس تحقیقات آزمایشگاهی و عددی پیشین انجام شده است.</span></span><br>
<strong><span style="font-family:Nazanin;"><span style="font-size:9.5pt;">یافتهها</span></span></strong>: <span style="font-family:Nazanin;"><span style="font-size:9.5pt;">مقدار تنش بهدستآمده در بخش جلویی دهانه داخلی گردن رحم در مدل پایه این تحقیق، بخشی که بیشترین تمرکز تنش و تغییر شکل در آن اتفاق میافتاد، مطابق پیشبینی مطالعات پیشین</span></span><span style="font-family:Nazanin;"><span style="font-size:9.5pt;"> حدود</span></span> <span dir="LTR">kPa</span><span style="font-family:Nazanin;"><span style="font-size:9.5pt;"> 5 میباشد. در سایر مدلها تنش مؤثر کمتر از این مقدار و در کمترین حالت </span></span><span dir="LTR">kPa</span><span style="font-family:Nazanin;"><span style="font-size:9.5pt;"> 3/5 است. میزان کرنش در مدل دهانه رحم با بافت کلاژنی نرم باردار و کیسه آمونیوتیک تغییر شکل یافته بیشتر از سایر مدلها بود، دلیل این امر اتفاق افتادن همزمان دو عامل ایجادکننده زایمان زودرس میباشد.</span></span><br>
<strong><span style="font-family:Nazanin;"><span style="font-size:9.5pt;">بحث و نتیجهگیری</span></span></strong>: <span style="font-family:Nazanin;"><span style="font-size:9.5pt;">نتایج این تحقیق نشان میدهد که تغییرات هندسه کیسه آمونیوتیک باعث افزایش بار بر روی دهانه رحم شده که باعث شروع فرآیند مخروطی شدن میشود. فرآیند مخروطی شدن فرآیندی است که در طی آن اتساع پیدا کردن اولیه دهانه رحم باعث تولید سیگنالهای شیمیایی توسط ماهیچههای صاف دهانه رحم برای اتساع بیشتر دهانه رحم و درنهایت نارسایی دهانه رحم میشود که یکی از مهمترین عوامل ایجادکننده زایمان زودرس است.</span></span><span dir="LTR"></span>
<a name="_Toc416081061"><strong><em>Background & Aims: </em></strong></a>Preterm labor is a complex process affected by several factors through which cervical failure plays a vital role in some patients. During the pregnancy, the proper cervical function is required to maintain the fetus in the uterus. Softness and shortness of the cervix are two main causes of preterm delivery. The aim of this study was to investigate the effect of the cervical softening and deformation of amniotic sac on mechanical function of the cervix under the organ mechanical environment.<br>
<strong><em>Materials & Methods: </em></strong>A 3D model of the uterus, cervix, and fetal membrane of a pregnant woman was built based on MR imaging in order to analyze the mechanical function of the uterus and cervix under physiological loading of pregnancy. In this study, to describe the collagenous tissue of the uterus and cervix, a hyperelastic composite material with a neo-Hookean ground substance assuming a continuous random fiber distribution was used. The effect of cervical remodeling on preterm delivery was studied using two types of fibers, pregnant, soft, and deformed, and non-pregnant, rigid and non-deformed. Also, the geometrical effects of amniotic sac have been studied by assuming two different geometries for amniotic sac which are deformed, and non-deformed. Behavior of tissue deformation resulted from stress, changes in the geometry of the organs and the interaction between the uterus, cervix and fetal membrane have been studied using finite element method and patient-specific geometry based on previous experimental and numerical investigations.<br>
<strong><em>Results: </em></strong>The amount of stress obtained at the front part of the internal mouth of the cervix of the basic model, the part where the highest concentration of stress and deformation occurred, as predicted by previous studies is approximately 5 kPa. In other models, the effective stress is less than this value, and is at least equal to 5.3 kPa. The strain rate in the soft cervical model and the deformed amniotic sac was higher than other models because both causes of early delivery exist simultaneously.<br>
<strong><em>Conclusion</em></strong><strong>: </strong>The present model shows that changes in the geometry of amniotic sac increase the load on the cervix and initiates the funneling. Funneling is a process in which the initial dilation of the cervix causes the production of chemical signals by the cervix smooth muscle cells causing further cervical dilatation and ultimately cervical insufficiency, which is one of the most important causes of preterm labor.
