نتایج حاملگی در زنان مبتلا به لوپوس اریتماتو سیستمیک در بیمارستان امام خمینی ارومیه

مقدمه: لوپوس اریتماتوسیستمیک یک بیماری اتوایمیون است که زنان را در سنین باروری مبتلا میکند. به همین علت متخصصان به بسیاری از مشکلات حاملگی در مبتلایان به لوپوس شامل تاثیر لوپوس بر پیامد های جنینی و همچنین حاملگی بر لوپوس توجه می کنند. این مطالعه جهت بررسی نتایج مرتبط با مادر و جنین در زنان حامله مبتلا به لوپوس طراحی شده است. موای و روش کار: این یک مطالعه گذشته نگر بر روی 20 حاملگی در 13 زن مبتلا به لوپوس اریتماتو سیستمیک دردپارتمان نفرولوژی دانشگاه علوم پزشکی ارومیه می باشد. نتایج: متوسط سن بیماران 24 سال (حد فاصل 19-37 سال) بود. 7 بیمار نولیپار بودند. 15 حاملگی از20 حاملگی در فاز رمیسیون و مابقی در فاز فعال بیماری اتفاق افتاده بود. در 5 بیمار بیوپسی کلیه قبل از حاملگی انجام شده بود. یک بیمار در مرحله 2، سه بیمار در مرحله 3 و یک بیمار در مرحله 4 نفریت لوپوسی قرار داشتند. عملکرد کلیه در تمام بیماران قبل از حاملگی طبیعی بود. 2 از 13 بیمار دارای تیترآنتی فسفولیپید آنتی بادی مثبت (APA) بودند اما فقط یکی از آنها سابقه سقطهای مکرر و ترومبوزوریدهای عمقی اندام تحتانی را داشت. در تمام بیماران APA مثبت از آسپرین و هپارین استفاده شد. در 7 مورد پروتیینوری در حد غیرنفروتیک در طی حاملگی مشاهده شد اما با اختلال در عملکرد کلیه، افزایش فشار خون و مورتالیتی مادر همراه نبود. تشدید بیماری در 3 مورد مشاهده شد که تاثیری بر پیش آگهی مادر و جنین نداشت. متوسط سن حاملگی 37 هفته (بین 34-39 هفته) با 5 مورد زایمان پره ترم بود. مرگ داخل رحمی جنین (IUFD) در2 مورد و سقط در یک مورد وجود داشت. متوسط وزن زمان تولد نوزادان 2160 گرم و آپگار 8 در زمان تولد بود. یک مورد اختلال رشد داخل رحمی (IUGR) و 3 مورد تولد نوزاد با وزن کم (LBW) گزارش شد. فقط یک مورد نوزاد LBW از مادر دارای بیماری فعال قبل از حاملگی متولد شده بود. تنها یک مورد از 5 حاملگی با تیتر مثبت آنتی فسفولیپید آنتی بادی ناموفق بود. غیر از یک مورد، کلیه بیماران در زمان لقاح تحت درمان با استرویید بودند. بحث: احتمال پیامدهای مطلوب مادر و جنین در حاملگی زنان مبتلا به لوپوس که مدت طولانی در رمیسیون کامل بوده، عملکرد کلیه نرمال و فشارخون طبیعی و تیتر منفی APA دارند زیاد است. اگرچه ریسک از دست رفتن جنین و / یا اختلال عملکرد کلیه در لوپوس فعال و/ یا نارسایی کلیه زیاد است.
<p> <strong><i>Background & Aim</i> <i>:</i> </strong>Systemic Lupus Erythematosus (SLE) is an autoimmune disease that affects primarily women, commonly in their reproductive years. Clinicians have to face the many problems of pregnancy in patients with SLE including the influence of SLE on fetal outcome and that of pregnancy on SLE. This study was performed to study maternal and fetal outcomes in women with SLE. </p><p> <strong><i>Material and Methods:</i> </strong>It is a retrospective study of 20 pregnancies in 13 women with SLE in nephrology department in Urmia University of Medical sciences. </p><p> <strong><i>Results:</i> </strong>The mean age of patients was 24 years old (ranges between 19–37). Seven patients were nulliparous. 15 of 20 pregnancies occurred in remission phase and others in active phase. In 5 patients kidney biopsy was performed before pregnancy. There was one patients on stage 2, three on stage 3 and one on stage 4 of lupus nephritis before pregnancy. The kidney function in all patients was normal before pregnancy. 2 of 13 patients had positive antiphospholipid antibodies (APA) but only one of them had past history of multiple abortion and deep vein thrombosis in lower exterimities. In all of APA positive patients aspirin and heparin were used. In 7 cases non-nephrotic range proteinuria was seen during pregnancy but no impairment of renal function, no hypertension and no maternal mortality was seen. In 3 cases flare up of SLE occurred during pregnancy with no influence on fetal and maternal outcome. The median gestational age was 37 weeks (range between 34 -39 weeks) with 5 preterm deliveries. Two Intrauterine Fetal Death (IUFD) and one abortion occurred. Mean birth weight was 2160 grams.The median Apgar score was 8 at birth.There was one IUGR and 3 Low Birth Weight (LBW). Only one LBW infant was from mother with active disease before pregnancy. Only one of 5 pregnancies in APA positive mothers was failed. Except one, all cases were on steroid treatment at time of conception. </p><p> <strong><i>Conclusion:</i> </strong>The probabilities of favorable fetal and maternal outcomes is high in women with stable and prolonged remission of SLE, normal renal function, normal blood pressure and negative APA. However, the risk of fetal loss and/or kidney function deterioration is high in patients with active SLE and/or renal insufficiency. Pregnancy planning during the inactive phase of renal disease, careful monitoring of blood pressure and renal function and treatment with Heparin and low dose aspirin in women with positive APA are the prerequisite for successful pregnancies in SLE. </p>
